29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. a. On the site where employees were performing demolition work, the employer did not provide training on the hazards of working above floor holes and demolition work thereby exposing the employees to a struck-by/fall hazard.

29 CFR 1926.855(b): Before demolishing any floor arch, debris and other material shall be removed from such arch and otheradjacent floor area. Planks not less than 2 inches by 10 inches in cross section, full size undressed, shall be provided for, and shall be used byemployees to stand on while breaking down floor arches between beams. Such planks shall be so located as to provide a safe support for the workmenshould the arch between the beams collapse. The open space between planks shall not exceed 16 inches. a. On the first floor of the building where employees removed concrete floor, the employees were standing on panel doors placed over the main beams while jack hammering the concrete floors thereby exposing them to a struck-by/fall hazard.

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey shall be made, by acompetent person, of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portionof the structure. Any adjacent structure where employees may be exposed shall also be similarly checked. The employer shall have in writing evidencethat such a survey has been performed. a. On the site the employer did not develop a demolition plan which clearly defined the means an methods for demolishing the structure and the equipment and material needed to perform that demolishtion safetly.
