29 CFR 1910.333(a)(1):  Live parts to which an employee could be exposed were not de-energized before the employee worked on or near them (work on or near de-energized parts is covered by paragraph 1910.333(b) of this section:  (a)  Continental Airlines Chelsea Food Services at 27550 E 75th Ave, Denver, CO 80249:  On and before September 8, 2011, the employer did not ensure that each employee working on or near exposed energized electrical panels, up to 480 volts, 3-phase, was protected from electrocution and arc flash hazards presented while pushing in the 24-volt latching relay on air conditioning units #1 and #3. These two units had not been de-energized in accordance with the Lockout/Tagout provisions of 1910.333(b), prior to allowing employees working on or near the electrical circuits. This condition exposed the employees to electrical shock and arc flash hazards.   (b)  Continental Airlines Chelsea Food Services at 27550 E 75th Ave, Denver, CO 80249:  On and before September 8, 2011, the employer did not ensure that each employee working on or near exposed energized electrical panels, up to 480 volts, 3-phase, was protected from electrocution and arc flash hazards presented while replacing fuses. Electrical equipment and panels are not de-energized in accordance with the Lockout/Tagout provisions of 1910.333(b), prior to allowing employees working on or near the electrical circuits. This condition exposed the employees to electrical shock and arc flash hazards.

29 CFR 1910.333(c)(2):  Non-qualified persons worked on energized electrical circuits or equipment that have not been de-energized under the procedures of paragraph (b) of this section:   (a)  Continental Airlines Chelsea Food Services at 27550 E 75th Ave, Denver, CO 80249:  On and before September 8, 2011, the employer did not ensure that each employee was qualified to work on electrical circuits or equipment that have not been de-energized, in that employees were exposed to electrical shock and arc flash hazards while pushing in a 24-volt latching relay on air conditioning units #1 and #3. The electrical panels were energized at 460 volts, 3-phase.   (b)  Continental Airlines Chelsea Food Services at 27550 E 75th Ave, Denver, CO 80249:  On and before September 8, 2011, the employer did not ensure that each employee was qualified to work on electrical circuits or equipment that have not been de-energized, in that employees were exposed to electrical shock and arc flash hazards while replacing fuses on electrical panels and equipment throughout the facility. The electrical panels and equipment are energized up to 480 volts, 3-phase.

29 CFR 1910.335(a)(1):   Employees working in areas where there were potential electrical hazards were not using electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed:   (a)  Continental Airlines Chelsea Food Services at 27550 E 75th Ave, Denver, CO 80249:  On and before September 8, 2011, the employer did not ensure that maintenance employees wore appropriate electrical protective equipment when performing voltage testing on energized 120 volt to 480 volt electrical circuits. The employees performed voltage testing without wearing rubber insulating gloves, and arc rated long sleeve shirt and arc rated pants, or arc rated coveralls with a minimum arc rating of 8, leather gloves, hard hat, and other required electrical protective equipment. This condition exposed the employees to electrical shock and arc flash hazards.   Abatement Note:  NFPA 70E, 2009 Edition, Table 130.7(C)(9) and Table 130.7(C)(10) categorize activities into risk groups, and describes protective clothing and personal protective equipment associated with each group. The criteria provides that if voltage testing is being performed on equipment energized at 480 volts, the personal protective equipment should satisfy an arc rating of 8 based on the Hazard/Risk Category of 2, and furthermore, discusses the need for rubber insulating gloves and other PPE.
