Instance #1: Procedures did not specifically state how to shut down, isolate, block and secure machines or equpment to control hazardous energy; b. Instance #2: Procedures did not specifically identify the placement, removal and transfer of lockout/tagout devices and the responsibility for them; c. Instance #3: Verification steps did not have enough specificity to verify the effectiveness of the energy control measures or how to accomplish this task.
