29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a)  Rol-Tech, Inc., Fort Loramie, Ohio: On or about September 13, 2011, the employer had not developed and implemented a energy control program to prevent unexpected start up, and protect employees from amputation hazards during the servicing and maintenance of the Pines Industrial hydraulic tubing benders located in the tubing department.    No additional abatement information is needed for this item.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Rol-Tech, Inc., Fort Loramie, Ohio:  On or about September 13, 2011, the employer had not effectively guarded the Pine Bender presses exposing employees amputation and crushing hazards at the pinch points between the ram die and the lower die, and the two lower die plates.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.212(a)(3)(ii):     Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    a)  Rol-Tech, Inc., Fort Loramie, Ohio:  On or about September 13, 2011, the employer had not effectively guarded the Pine Bender presses, exposing employees crushing hazards at the pinch points between the ram die and the pipe being bent.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1904.29(b)(1):     A log of all recordable work-related injuries and illnesses (OSHA 300 or equivalent), was not completed in the detail as required by the regulation:    Rol-Tech, Inc., Fort Loramie, Ohio: On or about September 13, 2011,  the employer did not complete an OSHA 300 in the detail required by the regulation.    a)  Employer injury and illness records provided by the employer were reviewed and indicated that on or about July 31, 2011, a recordable injury occurred when an employee was loading dies on a skid and their shoulder popped.  The injury resulted in employees activity being restricted for 10 days. The employer did not ensure that the object or substance that injured the employee was recorded in column F of OSHA log.    b)  Employer injury and illness records provided by the employer were reviewed and indicated that on or about July 28, 2011, a recordable injury occurred when an employee caught their hand and fingers in between the wings and ram die of the a tubing bender, crushing their hand and amputating three fingers.  The employer did not ensure that the number of days away was recorded in column K of OSHA log.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
