29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about August 4, 2011, employees responding to a chiller leak were potentially exposed to respiratory hazards from working with/around R123 refrigerant. Employees were not provided with appropriate respiratory protection for R123, which may result in light-headedness, dizziness, confusion, incoordination, drowsiness, or unconsciousness, and irregular heartbeat. Abatement Note: Abatement certification is required for this item.
