29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Pablo Marquez, 4150 Chesford Road, Columbus, Ohio: On or about September 15, 2011 at the work site located at 2417 State Route 60, Vermilion, Ohio, the employer did not ensure that employees received comprehensive training on the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while working on a roof. Employees did not use fall protection and were exposed to an approximately 15 feet fall hazard from the roof to pavement.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line systems and guardrail systems, warning line system and personal fall arrest system, or warning line system and safety monitoring system: (a) Pablo Marquez, 4150 Chesford Road, Columbus, Ohio: On or about September 15, 2011 at the work site located at 2417 State Route 60, Vermilion, Ohio, the employer did not ensure that employees who were performing roofing work on a 6/12 pitch roof approximately 12 feet above pavement were protected by a guardrail system, safety net system or used an appropriate fall arrest system. The employees were installing roofing material on a two-story hotel. (b) Pablo Marquez, 4150 Chesford Road, Columbus, Ohio: On or about September 15, 2011 at the work site located at 2417 State Route 60, Vermilion, Ohio, the employer did not ensure that employees who were performing roofing work on a 4/12 pitch roof more than 15 feet above pavement were protected by a guardrail system, safety net system or used an appropriate fall arrest system. The employees were installing roofing material on a two-story hotel. Pablo Marquez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315058313, citation number 2, item number 1, and was affirmed as a final order on or about 12/22/2010, with respect to a workplace located at 26945 Russell Road, Bay Village, Ohio. The Pablo Marquez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313156549, citation number 1, item number 1, and was affirmed as a final order on 06/08/2010, with respect to a workplace located at 1365 Briarcliff Road, Reynoldsburg, Ohio.
