29 CFR 1910.178(l)(3)(i)(A): Powered industrial truck operators did not receive, as part of their initial training, truck-related subject matter pertaining to operating instructions, warnings, or precautions for the type of truck the operator will be authorized to operate:           (a)     Niels Fugal Sons Company at 4265 E. Beaver Creek Blvd, Avon, CO 81620:  On and before September 13, 2011, the employer did not ensure all forklift operators were initially trained in operating instructions, warnings, or precautions such as, but not limited to, specific procedures related to unloading covered truck trailers.  This condition exposed employees to the hazards of unsafe forklift operations resulting in serious employee injury.    Note:  1926.602(d) Powered industrial truck operator training.  The requirements applicable to construction work under this paragraph are identical to those set forth at 1910.178(l) of this chapter.
