29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:    a)  Duran Roofing LLC at 507 Flagler Rd., Fort Collins, CO 80525:  On or before September 10, 2011, the employer did not ensure that employees were using fall protection when shingling a ranch style residential roof with a 6 in 12 pitch.  Condition exposed employees to fall hazards of up to 10 feet.    Abatement note:  Abatement certification and documentation are required for this item (See enclosed "Certification of Corrective Action Worksheet").    Duran Roofing was previously cited for a serious violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(13), which was contained in OSHA inspection number 313417883, citation number 1, item 1, issued April 7, 2010 and became a final order on May 27, 2010 with respect to a workplace located at 5608 Weeping Way, Fort Collins, CO 80528.    Duran Roofing was previously cited for a repeat violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(13), which was contained in OSHA inspection number 314657297, citation number 1, item 1, issued July 13, 2010 and became a final order on August 10, 2010 with respect to a workplace located at 326 Sandstone Drive, Johnstown, CO 80534.
