29 CFR 1910.132(a):     Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a)  On or about September 13, 2011, employee(s) washing windows were in close proximity to an unprotected edge of a second story roof without conventional fall protection equipment attached or equivalent.    (b)  On or about September 13, 2011, employee(s) lowering equipment, such as, but not limited to, ladders, from one level to another and in close proximity to unprotected edges were not protected from falling by conventional fall protection equipment.    (c)  On or about September 13, 2011, employee(s) accessed work locations in close proximity to unprotected sides and edges without being protected by conventional fall protection equipment attached.
