29 CFR 1910.335(a)(1)(i):     Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    (a)  In the testing area, employees performing testing activities were not provided with personal protective equipment to prevent contact with energized parts, exposing them to electrical hazards.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    (a)  In the testing area, the employer failed to ensure the exposed energized parts on the shop-made variac were guarded.  Employees were exposed to energized electrical hazards when using the variac for testing operations.

(a)  On or about 8/20/2009, injury #1 was not classified in columns G-J on the OSHA 300 Log.     (b)  On or about 8/3/2009, injury #2 was not classified in columns G-J on the OSHA 300 Log.     (c)  Injury #3 on the 2009 OSHA 300 Log occurred on or about 2/10/2010 and was recorded on the incorrect log.     (d)  Injury #4 on the 2009 OSHA 300 Log occurred on or about 2/13/2010 and was recorded on the incorrect log.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    (a)  On the shop-made variac used in the testing area, the incoming cord to the power control switch did not have strain relief to prevent the covering from being pulled away from the fitting, exposing employees to electrical hazards.
