29 CFR 1910.147(c)(4)(ii)(D):     The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.    On or about 9/21/11, lockout procedures for equipment including but not limited to exhaust fans, injection mold machines, vacuum pumps, air compressors, and condensate dryers did not include specific requirements for testing the equipment to determine and verify the effectiveness of energy control measures.

29 CFR 1910.176(b):     Storage of material created a hazard.    Employees working in the production area near raw material storage at line 5 were exposed to hazards from a loaded storage rack where a support column had been damaged thereby potentially reducing its capacity and creating the possibility of collapse of the damaged section. Boxes, each weighing approximately 800 to 1000 pounds, were stacked two high on the damaged rack.

29 CFR 1910.215(b)(9):     The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.    Tool & die area: On or about September 8, 2011, a Baldor brand grinder in the tool & die area had a tongue guard that was 1 and 3/16 inches from the grinding wheel.    OEM/Miller was previously cited for a violation of this occupational safety and health standard, 1910.215(b)(9), which was contained in OSHA inspection number 311472161, citation number 1, item number 2, and was affirmed as a final order on 3/18/2008, with respect to a workplace located at 1300 Danner Drive Aurora, OH 44202.
