29 CFR 1910.146(c)(8)(i):  When an employer arranged to have employees of another employer (contractor) perform work that involved permit space entry, the host employer did not inform the contractor that the workplace contained permit spaces and that permit space entry was allowed only through compliance with a permit space program meeting the requirements of this section:    The employer allowed contractors and their employees to enter permit spaces such as, but not limited to, the cyclone, scrubber and ductwork associated with the #4 boiler, when the entries did not comply with a program meeting the requirements of this section such as blocking, blanking, or misaligning the connecting ductwork to prevent the flue gases from the #1 and #2 boilers from entering the spaces occupied by the contractors and their employees.
