29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about 9/12/11 during construction activities including but not limited to installing electrical wiring and outlet boxes, the employer did not ensure holes measuring approximately 24 inches wide and 36 inches long were properly guarded to protect employees from fall hazards approximately 11 feet.
