29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) J. Alexander Roofing on or about September 13, 2011 at the address of 119 E. Lima St. in Findlay, OH:  The employer did not ensure employees were protected head injuries while working below employees performing roofing operations.  Employees tasked with clean-up operations were exposed to potential impact injuries called by falling material and/or tools.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) J. Alexander Roofing on or about September 13, 2011 at the address of 119 E. Lima St. in Findlay, OH:  The employer did not assure employees were protected from falls of approximately 13 feet while performing roofing work to a steep sloped roof.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

Lima St. in Findlay, OH:    The employer did not assure that employees were trained to recognize appropriate use of fall protection.  Employees performing roofing operations were exposed to a fall hazards of approximately 13 feet.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) J. Alexander Roofing on or about September 13, 2011 at the address of 119 E. Lima St. in Findlay, OH: Employees were exposed to falls of approximately 13 feet in that employees used an extension ladder to access the roof of structure without extending the ladders side rails at least three feet above the landing surface.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
