29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:       a)  A-Mark Roofing, located at worksite on 2816 High Point Lane in Findlay, OH:  On or about August 11, 2011, the employer did not ensure employees were protected from a fall hazard approximately 24' above the ground below.  The employees were placing new roofing material on a 5/12 pitched wood-framed residential structure.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:       a) A-Mark Roofing, located at worksite on 2816 High Point Lane, Findlay OH: On or about August 11, 2011, the employer did not ensure that employees were trained in the proper application of personal fall protection equipment while working on 5/12 pitched wood-framed residential structure.  Employees were exposed to a fall height approximately 24' from the ground below.
