29 CFR 1910.178(m)(5)(i): When powered industrial trucks were left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, and brakes were not set. a) PolyOne Corporation, Elk Grove Village, IL - On 9/22/11, an orange Toyota propane-driven powered industrial vehicle (forklift truck) was observed unattended, i.e., left idling with the brake lights on, by the cinder block wall near the exit door in Extrusion area that leads to the parking lot. In accordance with 29 CFR 1910.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1904.7(b)(4): When an injury or illness involved restricted work or job transfer, the OSHA 300 Log of recordable work-related injuries and illnesses did not have an entry of the total number of restricted or transferred days in the job transfer or restricted work days column. PolyOne Corporation, Elk Grove Village, IL - When an injury involved restricted work days, the employer did not enter the number of restricted work days in the appropriate corresponding column on the the 2010 OSHA 300 Log of Recordable Work-Related Injuries and Illnesses. a) Regarding Entry # 1, on or about March 11, 2010, an employee incurred an injury that resulted in restricted work activity of 75 days. Column I was checked to indicate work restriction, but Column L was left blank, and the number of restricted work days was not entered. b) Regarding Entry # 3, on or about December 1, 2010, an employee incurred an injury that resulted in restricted work activity of 30 days. Column I was checked to indicate work restriction, but Column L was left blank, and the number of restricted work days was not entered. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
