29 CFR 1910.24(f): The rise height and tread width shall be uniform throughout any flight of stairs: a. Outdoor Sorting Recycling Conveyor - On or about September 19, 2011, employees were required to access the conveyor by using two flight of stairs that had some steps not uniformly distanced. Employees were thereby exposed to slips, trips, and falls. The flights of stairs were the following; 1. The ascending flight of stairs had five (5) steps that were bent down and were not distanced at 6 inches per design. 2. The descending flight of stairs had six (6) steps that were bent down and were not distanced at 6 inches per design.

29 CFR 1910.23(a)(3): Where operating conditions necessitate the feeding of material into any hatchway or chute opening, protection was not provided to prevent a person from falling through the opening: a. Outdoor/ In feed Conveyor Work Station/North side - On or about September 19, 2011, employees were required to sort out recycling products from the conveyor line by using a "drop bin" for bricks (4 feet X 4 feet X 2 feet) that was not guarded with a hinged cover. Employees were thereby exposed to falls of 15 feet to the ground level. MARKHAM TRANSFER & RECYCLING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.23(A)(3) WHICH WAS CONTAINED IN OSHA INSPECTION # 3125 99814, CITATION # (1), ITEM (1), AND WAS AFFIRMED AS A FINAL ORDER ON NOVEMBER 20, 2009, WITH RESPECT TO A WORKPLACE LOCATED AT 5400 SOUTH LAWNDALE, IL 60525.
