B & H Construction worksite located at 7501 Hickory Valley Dr., Maumee, Ohio: On or about July 21, 2011, the employer did not ensure employees wore safety glasses while operating pneumatic nailers while performing framing activities on the two story residential structure. B & H Construction worksite located at 7501 Hickory Valley Dr., Maumee, Ohio: On or about July 21, 2011, the employer did not ensure employees wore safety glasses while operating electric circular saws while performing framing activities on the two story residential structure.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. A. B & H Construction worksite located at 7501 Hickory Valley Dr., Maumee, Ohio: On or about July 21, 2011, the employer did not ensure employees were protected from falls while installing rafters on the two story residential type structure. The employee was exposed to a fall distance of approximately 16.00 feet.
