29 CFR 1910.1030(c)(1)(i):     The employer having employee(s) with occupational exposure did not establish a written Exposure Control Plan designed to eliminate or minimize employee exposure:  a.  Decontamination Room - Noted on or about 9/8/11 and at times prior thereto, employees tasked with repairing and testing medical equipment contaminated with blood and other body fluids are potentially exposed to bloodborne pathogens.  An Exposure Control Plain has not been developed to include the following:  1.  An exposure determination was not made for employees, without regard to the use of personal protective equipment, who are potentially exposed to bloodborne pathogens.  2.  The schedule and method of implementing paragraph (d), methods of compliance, did not specify that universal precautions shall be observed to prevent contact with blood or other potentially infectious materials.  3. The offering of the Hepatitis B vaccination series and post-exposure evaluation and follow-up as required.  4. Communication of the hazards to employees.  5. Recordkeeping requirements, specifically for training and medical documentation.  Additionally, if an employee declines receiving the Hepatitis B vaccine series, declination forms must also be maintained.    *Note:  The Exposure Control Plan shall be reviewed and updated at least annually.

29 CFR 1910.1030(f)(1)(i):   The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure:  a.  Decontamination Room - On or about 9/8/11 and at times prior thereto, employees tasked with decontaminating medical equipment are potentially exposed to bloodborne pathogen hazards and have not been offered the Hepatitis B vaccination series.

29 CFR 1910.1030(h)(2)(i):   The employer's records for annual bloodborne pathogen training did not include all of the elements required by (h)(2)(i)(A)-(D) of 29 CFR 1910.1030:  a.  Decontamination Room - Noted on or about 9/8/11 and at times prior thereto, the employer did not maintain training records that included the following information:  1.	The dates of the training session;  2.	The contents or a summary of the training session;  3.	The names and qualifications of persons conducting the training;  4.	The names and job titles of all persons attending the training session  Note:  Training records shall be maintained for 3 years from the date on which the training occurred.

29 CFR 1910.1200(f)(5)(i):     The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein:  a.  Tube Room - Noted on or about 9/8/11 and at times prior thereto, a 30-gallon drum containing spent mineral oil was not labeled with the identiity of its' contents.
