The employer did not ensure that sodium hypochlorite and acids were used and stored in a manner that would prevent their accidental mixing.     (a)  Chemical Feed Trailer - The fibergalss sub-floor catches spills/leaks of both sodium hypochlorite and hydroxyethlidenediphosphonic acid.    (b)  Chemical Feed TrialerTank Farm - Storage tanks of sodium hypochlorite and hydroxyethlidenediphosphonic acid are in the same bermed area and share the same drainage system.    (c)  Cooling Towers Water Treatment Tank Farm -  Storage tanks of sodium hypochlorite and phosphonic acid are in the same bermed area and share the same drainage system.
