29 CFR 1915.77(c): When employees were working aloft, or elsewhere at elevations more than 5 feet above a solid surface, either scaffolds or a sloping ladder, meeting the requirements of Subpart E- Scaffolds, Ladders, and Other Working Surfaces, were not provided to afford safe footing, and the employees were not protected by safety belts and lifelines meeting the requirements of 29 CFR 1915.159 and 1915.160. (a) Aboard the vessel Knud Lauritzen, Port Hueneme, CA - On or about August 22, 2011, 1533 hours, An employee was working on the SATCOM mast of the vessel, the employee was standing on the mid-rails of the guard railing exposing himself to falls of approximately 30 feet to the next lower level, the employee was not provided safe footing or protected by a fall restraint system.
