29 CFR 1904.39(a): Within eight (8) hours after the death of any employee from a work-related incident or the in-patient hospitalization of three or more employees as a result of a work-related incident, the employer did not orally report the fatality/multiple hospitalization by telephone or in person to the Area Office of the Occupational Safety and Health Administration: a) U.S. Army, Fort Huachuca - Fort Huachuca, AZ: The fatality of a Civilian Police Officer which occurred on August 18, 2011 at approximately 10:23 AM was not reported to the Occupational Safety and Health Administration (OSHA) until August 19, 2011 at approximately 9:30 AM.
