29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about September 20, 2011 and at times prior thereto, employees were exposed to a fall of approximately 6 feet 4 inches into an unprotected steam tunnel.

29 CFR 1926.701(b): Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement. On or about September 20, 2011 and at times prior thereto, employees were exposed to impalement from the rebar approximately 18 inches in length while walking and working in the area along all sides of the steam tunnel.

29 CFR 1926.1412(d)(1)(v): The hooks and latches were not inspected by a competent person for deformation, cracks, excessive wear, or damage such as from chemicals or heat: On or about September 20, 2011 and at times prior thereto, employees were exposed to being struck by and crushed by steel columns, I beams and bar joist being elevated by the 50 ton American Crane which had a defective safety latch.
