29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a)  Davalos, Maria dba Davalos Roofing at 1470 N. Birch Street, Denver, CO 80220:  On or before September 10, 2011, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns.  Condition exposed employees to the hazard of projectiles to the eyes.    Abatement note:  Abatement certification is required for this item (see enclosed, "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:    a)  Davalos, Maria dba Davalos Roofing at 1470 N. Birch Street, Denver, CO 80220:  On or before September 10, 2011, the employer did not ensure that employees were using fall protection when shingling the roof with a 6 in 12 pitch.    Condition exposed employees to fall hazards of up to 11 feet 8 inches.    Abatement note:  Abatement certification is required for this item (See enclosed "Certification of Corrective Action Worksheet").
