29 CFR 1910.37(b)(4):   Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  a)   The warehouse storage rows with various cross walkways and blocked areas did not contain clearly visible exit routes or signs indicating the way to the nearest exit.

29 CFR 1910.37(b)(5):   Each doorway along the exit routes in the office/breakroom areas of SARSS that could be mistaken as an exit was not marked "Not an Exit" or identified by a sign indicating its actual use (e.g. closet):  a)  The main hallway leading to the nearest exit contained a closet and office that were not not marked as "not an exit" or by a sign indicating their actual use and could be confused with a route to an exit.  b)  Doors to a restroom and storage closet along the designated exit route through the breakroom did not have signs stating "Not an Exit" or indicating their actual use.  A door across the room from the designated exit route leading to the changeroom also did not contain a "Not an Exit" sign or indicate its actual use although it looked similar to the exit door out of the buidling.

29 CFR 1910.305(b)(2)(i):   All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  Pull boxes, junction boxes, and fittings were not provided with covers identified for the purpose.  a)  A junction box containing live wires for the lighting system was not covered on August 12, 2011.

29 CFR 1904.30(a):   Basic requirement. You must keep a separate OSHA 300 Log for each establishment that is expected to be in operation for one year or longer.  a)  SARSS #1 Stockton did not keep OSHA 300 log specific to it's facility to identify injury and illness trends associated with it's specfiic worksite during calendar years 2008, 2009, 2010 and 2011.  SARSS combined it's log with the Stockton CSMS unit.
