29 CFR 1926.20(b)(2):     The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    On or about September 22, 2011 Robinson Roofing & Sheet Metal, Inc. did not initiate a program or provided a competent person to conduct frequent site  inspections, exposing employees to the hazards associated with construction work.    In accordance with CFR 29 1903.19(c) abatement certification is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.21(b)(1):     The employer should avail himself of the safety and health training programs the Secretaryprovides.    On or about September 22, 2011 Robinson Roofing & Sheet Metal, Inc. did not provide employees a safety and health program to educate and train employees of the hazards associated with construction work.    In accordance with CFR 29 1903.19(d) abatement documentation is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.453(b)(2)(v):     A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated.    On September 22, 2011 two employees were not protected from a fall while working from an aerial lift, exposing them to a fall of approximately 40 feet.    Abatement documentation or certification is not required for this item.
