29 CFR 1926.501(b)(4)(i):     Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.      On December 21, 2011 employees were not protected from a fall as they cut and worked around a skylight opening in a roof deck, exposing them to a fall hazard over over 25 feet.     In accordance with CFR 29 1903.19(d) abatement documentation is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.502(f)(2)(i):       The rope, wire or chain was not flagged at intervals of 6- feet (1.8 m) or less with high visibility material.      On December 21, 2011 employees were not protected from a fall by a warning line that was flagged at intervals of 6 feet or less, exposing employees to a fall hazard of over 25 feet.    In accordance with CFR 29 1903.19(c) abatement certification is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      On December 21, 2011 employees used an extension ladder that did not extend three feet past the upper landing, exposing them to a fall hazard of over 25 feet.    In accordance with CFR 29 1903.19(c) abatement certification is required of this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
