29 CFR 1910.178(a)(4): Modifications and additions which affected capacity and safe operation were performed without manufacturers written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly. (a) Employees were potentially exposed to serious injury from the use of a forklift that had modified forks (holes welded or drilled into the tips) without the manufacturer's written approval and without a change to the capacity, maintenance instruction plates, tags or decals. The forks were on the Hyster H60XM forklift, vehicle #931-0691 located at the warehouse at 59973 Downs Road in La Grande, OR.
