29 CFR 1926.350(a)(10): Oxygen cylinders in storage were not separated from fuel-gas cylinder or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour. On or about September 21, 2011, employees were exposed to burns due to possible fires next to two oxygen cylinders which were stored next to acetylene cylinders without a fire-resistance barrier with a rating of at lest one-half hour.

29 CFR 1926.1412(d)(1): A visual inspection by competent person(s) was not conducted prior to each shift the equipment was used. Apparent deficiencies in the items noted in 1926.1412(d)(1)(i) thru (xiv), at a minimum, were not observed: On or about September 21, 2011, employees were exposed to a crushing hazard from falling loads while working with a Feterl GEN 11 10000 mobile hydraulic crane. The employer failed to ensure that an adequate visual inspection of the equipment was made by the competent person prior to each shift.
