29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). (a) Moltz Construction, Inc. @ 401 7th St., Glenwood Springs, CO 81601: On 9/21/11, the employer did not ensure that employees were protected from cave-ins. An employee was observed in an excavation outside of the trench box. The depth of the trench, as measured near the ladder access on the south side of the excavation, was 5.3ft. This condition exposed the employee to a cave-in hazard.
