29 CFR 1910.146(c)(7)(iii): The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a certification that contained the date, location of the space, and the signature of the person that made the determination while making available to each employee entering the space or to that employees authorized representative: At the BOF ladle re-line pits where the employer reclassified a permit-required confined space to a non-permit confined space where no atmospheric hazard existed and the non-atmospheric hazard had been eliminated through energy control procedures without entry into the spaces; the employer failed to provide written certification to each employee entering the space or to the employees authorized representative. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
