29 CFR 1926.651(c)(2): Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a) At the construction site in Canal Winchester, Ohio the employer had two employees working in an excavation, approximately 8 feet in depth, within a trench shield box, without any means of egress from the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). a) At the site where employees were not adequately protected while working in an excavation on a water main at the Lifestyles Communities apartment complex in Canal Winchester, Ohio. The employer had employees working within a trench shield system, approximately 8 feet in depth, with no protection from cave-in on either side of the shield system where the 4 foot struts separated the two sidewalls of the trench shield system.
