29 CFR 1926.754(e)(2)(ii): Roof and floor holes and openings were not decked over. Where large size, configuration or other structural design does not allow openings to be decked over (such as elevator shafts, stair wells, etc.) employees shall be protected in accordance with 1926.760(a)(1): (a) On the second level of the structure at Gridline 8 between Columns F and B on or about September 13, 2011, and at times prior thereto, 9 inch by 5.75 feet floor holes and 2.5 feet by 5.75 feet floor openings were not decked over. Iron workers placed the bottom of extension ladders about 3 to 3.5 feet from the holes and openings and used the ladders to install bridge work and angle iron at the roof level, about 13 feet, 8 inches above the openings. It was about 13 feet 8 inches from the second level deck holes and openings to the concrete floor below. Barrier tape, placed horizontally at the edge of the holes and openings, was used to warn employees of the fall hazard. Reportedly, the deck holes and openings were not covered because ultrasonic tests were to be conducted on the structural steel welds.
