29 CFR 1926.416(e)(1):     Worn or frayed electric cords or cables shall not be used:    (a) Inside the structure on or about September 12, 2011, and at times prior thereto, the outside insulation was pulled away from the female plugs/receptacles on two, 220 VAC branch circuit cords that supplied power to two, separate spider boxes.  Soltek's foreman said they were responsible for these cords.
