In energized installations, each outlet box did not have a cover, faceplate, or fixture canopy:    a) Jobsite: On September 21, 2011 and times prior therto the controlling employer, Cotner Building Company, LLC, did not ensure it's sub-contractors protected their employees from an energized circuit breaker panel in the garage that was covered with cardboard and cut out to allow exposure/easy access to the energized circuit breakers inside.      b) Jobsite: On September 21, 2011 and times prior therto the controlling employer, Cotner Building Company, LLC, did not ensure it's sub-contractors protected their employees from two energized outlets in the garage that did not have coverplates installed.     Note) Abatement certification IS required for this item.
