In energized installation(s), each outlet box did not have a cover, faceplate, or fixture canopy:    a) Jobsite: On September 21, 2011 and times prior thereto the creating employer, George's Electirc, Inc., did not install a proper cover on the circuit breaker panelboard in the garage, exposing employees to the energized circuits inside.      b) Jobsite: On September 21, 2011 and times prior thereto the creating employer, George's Electirc, Inc., did not install face plates on two outlets in the garage, exposing employees to the energized circuits.     Note) Abatement certification IS required for this item.
