29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):    (a) Liberty Roofing @ 62927 US Hwy 40, Granby, CO 80446: On 9/20/11, the controlling employer did not ensure that safety inspections regarding the use of personal fall arrest systems were conducted. Subcontracted employees conducted roofing activities on a 5 in 12 pitch roof without fall protection. This condition exposed four subcontracted employees to a fall hazard in excess of 25ft.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented.                                                                                                               (a) Liberty Roofing @ 62927 US Hwy 40, Granby, CO 80446: On 9/20/11, the controlling employer did not ensure that subcontracted employees worked on a 5 in 12 pitch multi-family residential roof with personal fall arrest systems. This condition exposed four subcontracted employees to a fall hazard in excess of 25ft.
