29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about September 22, 2011 employees were working on a roof around and installing skylights performing roofing work without fall protection exposing employees to fall hazards. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c)

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. On or about September 22, 2011 employees were working on a roof performing roofing work without fall protection exposing the employees to a fall hazard of 22.3 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
