(a) Auditorium -The employer did not have a competent person train the employees engaged in erecting scaffolding in the Auditorium in the following: the nature of scaffold hazards; the correct procedures for inspecting and erecting the tubular welded scaffold (including its components); the scaffold's design criteria, maximum intended load-carrying capacity and intended use; and the installation and use of fall protection during the scaffold erection.
