29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including but not limited to, isolating the permit space, and verifying that conditions in the permit space were acceptable for entry throughout the duration of the authorized entry: (a) At the Wausae Paper facility in Middletown, Ohio, the employer did not ensure that equipment and ductwork associated with their coal powered boilers was isolated using methods such as, but not limited to, blocking, blanking, or misaligning the connecting ductwork, to prevent boiler combustion gases from entering the confined spaces occupied by their employees. (b) The entry permits did not verify that acceptable conditions such as oxygen, LEL, and toxic gas levels were maintained throughout the duration of the authorized entry.
