29 CFR 1926.651(c)(2):     A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    On or about 9/26/11 during construction activities including but not limited to installing a water line in a trench approximately 200 feet in length and 30 inches wide, the employer did not ensure employees working in the trench making water line connections were provided a proper entry/egress. There was no ladder located in the trench and an employee was observed climbing in and out of the trench.
