29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about 9/26/2011, during construction activities including but not limited to roofing activities, the employer did not ensure employees were protected from fall hazards approximately 12 feet by ensuring a fall protection system/device was being used.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder���s length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder: On or about September 26, 2011, during construction activities including but not limited to roofing activities on a low sloped roof, the employer did not ensure employees using a ladder to access the roof were protected from fall hazards approximately 12 feet, by ensuring the ladder extended at least three feet above the roof surface or a grab rail was provided for mounting/dismounting the ladder.
