29 CFR 1910.218(a)(3)(iv): Blocks or wedges were not made of material the strength and construction of which met or exceeded the specifications and dimensions shown in Table O-11.    On or about September 22, 2011, employees used 2" x 4" wood boards to prop up rams and dies of 4,000 pound gravity drop forge hammers during servicing and maintenance procedures.  The boards did not meet the minimum allowable strength and dimension requirements for use in forging machines.     No abatement documentation or certification is required for this item.

29 CFR 1910.218(a)(3)(ix): Scale guards of substantial construction, arranged to stop flying scale, were not provided at the back of forging press(es):    Scale guards made of sheet metal and used at the back of the trim presses were not adequate to prevent employees from being struck by hot, flying scale.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.218(b)(2): The foot operated device(s) of forging machine(s) were not substantially and effectively protected from unintended operation:    On or about September 22, 2011, the guard covering the foot pedal for Hammer No. 9 did not substantially and effectively protect the unintended operation of the hammer in that the operator could step on the guard and the guard would depress the foot pedal, activating the hammer.  The hammer operator was exposed to an amputation hazard by the unintended hammer activation.    No abatement documentation or certification is needed for this item.

29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness.     On or about September 22, 2011, the employer did not use an equivalent form as the OSHA Form 301 to document a work related injury.  The Supervisor's Injury Investigation/Loss Source Identification form did not collect and record the following information as collected on the OSHA 301:      1.  Employee's address, date of birth, date hired, and gender;    2.  The name of the treating health care professional, treatment facility name and address;    3.  If the employee was treated in the emergency room;    4.  If the employee was hospitalized overnight as an in-patient;    5.  A description of what the employee was doing just before the incident occured;    6.  A description of what happend that lead to the incident.    In accordance with 29 CFR 1903.19(d), abatement documentation is required for this item.
