Condition exposed employees to the hazard of projectiles to the eyes. Abatement note: Abatement certification is required for this item (see enclosed, "Sample Abatement-Certification Letter").

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet(1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems: a) Caceres, Bernardo dba Berna Roofing at 703 Hillrose Court, Fort Collins, CO 80525: On or before September 24, 2011, the employer did not ensure that employees were using fall protection when shingling a two story residential roof with a 4 in 12 pitch. Condition exposed employees to fall hazards of up to 20 feet. Abatement note: Abatement certification and documentation are required for this item (See enclosed "Certification of Corrective Action Worksheet"). Berna Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(13), which was contained in OSHA inspection number 313412926, citation number 1, item 1, issued September 3, 2009 and became a final order on September 30, 2009, with respect to a workplace located at 1423 Mallard Drive, Johnstown, CO 80534.
