29 CFR 1926.651(i)(3): Sidewalks, pavements and appurtenant structure shall not be undermined unless a support system or another method of protection is provided to protect employees from the possible collapse of such structures.    a.  On or about September 16, 2011, at the above addressed jobsite, employees installing a sewer undermined the pavement by approximately three (3) feet, thereby exposing employees to the hazards associated with a street collapse.    THE ERM PLUMBING, INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.651(i)(3), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313935892, CITATION NUMBER 1, ITEM NUMBER 2b AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 29, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 1300 WESTERN AVENUE, CHICAGO HEIGHTS, IL 60411.

29 CFR 1926.652(a)(1):  Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:    a. On or about September 16, 2011, at the above addressed jobsite, employees were installing a sewer in a trench measured at six (6) feet seven (7) inches in depth.  The trench was not provided with cave in protection, thereby exposing the employees to cave in hazards.    THE ERM PLUMBING, INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313935892, CITATION NUMBER 1, ITEM NUMBER 2c AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 29, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 1300 WESTERN AVENUE, CHICAGO HEIGHTS, IL 60411.
