29 CFR 1926.416(a)(1):  Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a)  On or about August 3, 2021, employees were exposed to electrical shock while working around an underground electrical primary line carrying a voltage of 12kv.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. ERM Plumbing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a) which was contained in OSHA inspection number 1468653, citation 1, item 1, and was affirmed final order on August 18, 2020, with respect to a workplace located at 3648 N Fremont St, Chicago, IL 60613.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. ERM Plumbing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 1468653, citation 2, item 1, and was affirmed final order on August 18, 2020, with respect to a workplace located at 3648 N Fremont St, Chicago, IL 60613.  ERM Plumbing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection number 1260035, citation 1, item 1, and was affirmed final order on November 8, 2017, with respect to a workplace located at Tower and Burr, Winnetka, IL 60093.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
