29 CFR 1910.141(a)(5):     Every enclosed workplace was not constructed, equipped, and maintained, so far as reasonably practicable, as to prevent the entrance or harborage of rodents, insects, and other vermin:    a) At Bldg. 22143, Break room - On or about 8/17/11, a screen door with a gap at the hinges did not effectively prevent the entrance of rodents.    b) At Bldg. 22143, Inventory section, Director's office -  On or about 8/17/11, a gap was found in a window that did not effectively prevent entrance of rodents.    c) At Bldg. 22143, throughout facility - On or about 8/17/11, and at times prior thereto, an ongoing regular cleaning and maintenance of the HVAC and false ceilings was not conducted so as to minimize harborage of rodents.
