29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On 09/28/2011, employer did not develop and utilize procedures for the control of potentially hazardous energy which powered rotating rollers along the ceiling of Dryer 1 which were not turned off or shut down while entrant employees were cleaning the rollers which exposed the employees to entanglement hazards. IN THE ALTERNATIVE 29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: On 09/28/2011, machine guarding was not provided to protect entrant employees while cleaning rollers in Dryer 1 from entanglement hazards created by rotating rollers located along the ceiling of Dryer 1. ABATEMENT CERTIFICATION AND ABATEMENT DOCUMENTATION REQUIRED
