29 CFR 1915.71(j)(3):� - When rails were omitted, employees more than 5 feet above solid surfaces were not protected by safety belts and life lines that met the requirements of 1915.159 and 1915.160. USCGC Bertholf, Coast Guard Island, Alameda, CA: On September 29, one employee was not tied in, and the open hole inside the scaffold was not protected by guardrails. The employee was exposed to a fall of 12 feet through the scaffold opening, including but not limited to when he was passing down planks through the opening, walking around the scaffold, and removing portions of the scaffold.
