29 CFR 1910.27(d)(2): When ladders were used to ascend to heights exceeding 20 feet, and a cage was installed around the ladder way, a landing platform was not provided for each 30 feet of height or fraction thereof:     On or about September 19, 2011 an employee ascended a fixed metal ladder leading to the head of the dryer leg to assist with maintenance operations. The ladder was forty-four (44) feet high long and not equipped with a rest-platform at heights of thirty (30) feet or fraction thereof, exposing the employee to a fall hazard.

29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    a) The flexible cord powering the aeration fan on Bin No. 4 was damaged inside fan encasement next to the encasement surface, exposing employees to a potential electrocution or shock hazards from the exposed conductors.     b) The flexible cord powering the aeration fan on Bin No. 7 was pulled away from the junction box and cut in two seperate areas along the sheathing, exposing the conductors. Employees were exposed to a potential electrocution of shock hazard.
